This form is for the convenience of students who have participated in the Assessment of Prior Learning program.

This is not a CCV request form.

Students who have successfully completed the 7-10 month intensive Assessment of Prior Learning (APL) program, and who have received Vermont State Colleges Office of External Programs credit, may use the request form below to transfer their APL credit.
