APL Credit Transfer Request Form

This form is for the convenience of Prior Learning alumni. Please use this form only if you have participated in this program.

If you are a traditional CCV student, please do not use this form, but search further to find CCV's transcript request form, which has CCV's logo at the top.

Students who have successfully completed the 7-10 month intensive Assessment of Prior Learning (APL) program, and who have received Vermont State Colleges Office of External Programs transfer credit, may use the request form below to have their APL transcript sent to another college to be assessed for transfer.
